Understanding Damp-Cold
Dampness is heavy and sluggish; Cold slows everything further. Together they settle in the middle and the limbs, leaving a picture of heaviness, coldness and congestion rather than sharp pain or heat.
The body and limbs feel heavy, the limbs are cold and there is an aversion to cold. The stools are loose or diarrhoea, and the abdomen may feel full and cold.
The tongue is pale, swollen and teeth-marked with a thick white greasy coat and an excessively moist surface. The pulse is deep, slow and often slippery.
It follows cold damp environments, cold and raw food, an underlying Spleen Yang weakness, or a rich and greasy diet.
Treatment warms and dries. Aromatics that transform Dampness, warming herbs and moxibustion are the core, and cold food undoes the treatment.

Better with
- Warmth and warm environments

Timing
- Symptoms are worse in cold damp weather and after cold food.
- They improve with sustained warmth.
Practitioner notes
- The tongue is pale, swollen and teeth-marked with a thick white greasy coat and a wet surface.
- The pulse is deep, slow and slippery.
- The distinction from Damp-Heat is colour and temperature: here white and cold, there yellow and hot.
- The distinction from Spleen Yang deficiency is the thickness of the coat: this is an active obstruction on top of any deficiency.
Causes
Common causes
- Exposure to cold damp environments
- Cold and raw food
- Underlying Spleen Yang deficiency
- Greasy, sweet or rich diet
How it develops
- It can be acute after exposure, or chronic from diet and environment.
- If it persists, it deepens the Spleen Yang deficiency underneath.
Diagnosis
Diagnosis rests on the pale wet greasy tongue with a heavy cold body.
Tongue
- Body: Pale, swollen and teeth-marked.
- Coating: Thick white greasy, and excessively moist.
- Note: A yellow coat would indicate Damp-Heat.
Pulse
Deep, Slow, Slippery
Deep slow slippery fits interior cold dampness.
Observation
- Pale, puffy face
- Warm clothing
- Heavy movement
Listening
- A soft, slow voice
- Abdominal gurgling
Palpation
- Abdomen: Cool and full, improved by warmth.
- Channels: BL-20 and ST-40 may be sensitive; the abdomen feels cool.
Questions that separate it from look-alikes
- Are the stools loose?
- Is there thirst, and for warm or cold drinks?
- How much cold and raw food?
- Does warmth help?
Treatment
Principles
- Warm and dry Cold-Damp
- Transform Dampness with aromatics
- Support the Spleen so Dampness does not reform
- Avoid cold and greasy food
Herbs
Formulas
- Ping Wei San - The reference formula for Dampness obstructing the middle, drying and moving.
- Hou Po Wen Zhong Tang - For Cold-Damp with fullness and diarrhoea, warming and drying.
- Wei Ling Tang - For cold dampness with fluid retention and loose stools.
- Shi Pi Yin - For Spleen Yang deficiency with swelling and fatigue.
- Shen Fu Tang - For deeper Yang deficiency with coldness; a stronger warming formula.
- Li Zhong Wan - The middle-warming formula for cold deficiency.
Acupoints
- ST-40 (Fenglong) - The point for transforming Dampness and Phlegm.
- SP-6 (Sanyinjiao) - Drains Dampness and supports the Spleen.
- REN-4 (Guanyuan) - Warms the lower abdomen and strengthens Yang.
- BL-20 (Pishu) - The Spleen back-shu point, used for cold dampness of the middle.
Personalising the plan
- If cold dominates, warming herbs and moxibustion are increased.
- If Dampness dominates, aromatic transforming herbs are increased.
- If the Spleen Yang weakness is deep, warming tonics are added.
- If there is nausea, descending aromatic herbs are added.
Self-care
Diet
- Warm cooked food; ginger, cinnamon and small amounts of pepper
- Avoid cold, raw and iced food and drink
- Reduce dairy, sugar and greasy food
Lifestyle
- Keep warm and dry; apply warmth to the abdomen
- Moxibustion is traditionally used
- Gentle exercise to move Dampness
- Avoid damp living spaces
Qigong and movement
- Gentle warming practice emphasising the abdomen.
How it differs from look-alikes
Patterns that can be mistaken for damp-cold, and the contrast that separates them.
Damp-Heat
A red tongue with a yellow coat and heat signs. This pattern is pale, white-coated and cold.
Spleen Yang Deficiency
The underlying cold deficiency; here there is an active thick-coat obstruction.
Phlegm
Phlegm is thicker and more congealed, producing copious phlegm and nodules rather than general heaviness.
Wind-Cold-Damp
That pattern centres on the channels and joints with wandering pain; this one is general and middle-centred.
Where it appears
Skin conditions in which this pattern is commonly seen.
Safety and when to get help
For anything severe, sudden or spreading quickly, read when a skin problem needs urgent care.
This page describes a TCM pattern, not a diagnosis, and a pattern can accompany more than one skin condition. Get urgent medical help for a rash with fever, confusion or feeling very unwell; skin that is blistering, peeling or spreading quickly; swelling of the face, lips or throat, or difficulty breathing; or a lesion with increasing pain, warmth, red streaks or pus. For anything persistent, severe or changing, have it assessed by a clinician before treating it yourself.
Read the skin condition pages for the red flags that belong to a specific presentation.
Urgent-sign guidance is based on standard sources: NHS anaphylaxis, NHS cellulitis, NHS Stevens-Johnson syndrome, NHS melanoma.
Evidence note
No controlled trial of this treatment in skin conditions was identified for this entry. What is described here rests on the classical and later materia medica record and on traditional clinical use, not on controlled trial evidence, and it is not a prediction of benefit. Where skin-specific research does exist it is linked above.
Common questions
Why is the tongue coat so thick?
A thick greasy coat shows Dampness directly. Its white colour and wetness show that Cold is present rather than Heat.
Why does warmth help?
Cold contracts and Dampness congests; warmth opens and moves both. That is also why moxibustion is used.
Why avoid cold food?
Cold food adds Cold to a system already unable to warm and dry itself, so it worsens the heaviness and the loose stools.
When should I see a doctor?
Persistent diarrhoea, weight loss or blood in the stool needs medical assessment.
