Understanding Phlegm-Dampness in the Middle-Burner
The middle burner is where food is received and transformed. When Phlegm and Dampness obstruct it, food does not descend properly and the whole middle feels congested.
Fullness and stuffiness in the upper abdomen are the leading signs, often worse after eating. Nausea or vomiting may follow, and appetite is poor.
The body and limbs feel heavy, the head feels muzzy, and the stools may be loose or sticky. The tongue is pale, swollen and teeth-marked with a thick white greasy coat.
It comes from greasy sweet rich food, raw and cold food, Spleen deficiency from overwork or illness, and damp living conditions.
Treatment transforms Phlegm, dries Dampness and moves the middle. Rich tonics are withheld while the coat is thick, because a congested middle cannot process them.
Signs and symptoms
Key signs
- Fullness and stuffiness in the upper abdomen
- Nausea or vomiting
- Heavy sensation in the body and limbs
- Greasy sticky tongue coating
Also seen
- Poor appetite
- A muzzy head
- Loose or sticky stools
- Belching
- Sleepiness after eating
- A sticky taste in the mouth
Variations
- Vomiting of clear fluid
- Dizziness with nausea
- A feeling of food sitting in the stomach
Worse with
- Greasy, fatty or fried food
- Overeating or eating too quickly
- Dairy, cold and raw food
Better with
- Warm light food
- Smaller meals
- Gentle movement after eating
Timing
- Symptoms build after meals and in damp weather.
- They ease with a light diet over several days.
Practitioner notes
- The tongue is pale and swollen with teeth marks, a thick white greasy coat and an excessively wet surface.
- The pulse is slippery, often wiry or slow.
- The distinction from Spleen Qi deficiency is the coat: deficiency has a thin coat, this has a thick greasy one.
- The distinction from Damp-Heat is colour and thirst: here white and no thirst for cold drinks.
Causes
Common causes
- Greasy, sweet or rich food
- Raw and cold food
- Spleen Qi deficiency from overwork or chronic illness
- Living in a damp environment
How it develops
- It develops slowly from dietary strain on a weak Spleen.
- If it persists, it can affect the head with dizziness and heaviness.
Diagnosis
Diagnosis rests on the thick greasy coat with middle fullness and nausea.
Tongue
- Body: Pale and swollen with teeth marks.
- Coating: Thick white greasy and wet.
- Note: A yellow coat would indicate Damp-Heat.
Pulse
Slippery, Wiry, Slow
Slippery is the Phlegm quality.
Observation
- A heavy, puffy appearance
- A coated tongue visible on opening the mouth
- A slow bearing
Listening
- Belching
- Reports of nausea
Palpation
- Abdomen: Fullness and mild tenderness in the upper abdomen without guarding.
- Channels: REN-12 may feel full; the abdomen may feel cool.
Questions that separate it from look-alikes
- Is there fullness or nausea after eating?
- How are the stools?
- What is the diet like, especially greasy and cold food?
- Is the head muzzy or heavy?
Treatment
Principles
- Transform Phlegm and dry Dampness
- Move Qi and settle the middle
- Restore appetite and normal descent of food
- Avoid rich tonics while the coat is thick
Herbs
Formulas
- Er Chen Tang - The reference formula for transforming Damp-Phlegm.
- Ping Wei San - For Dampness obstructing the middle, drying and moving.
- Liu Jun Zi Tang - For the Spleen deficiency underneath the Phlegm.
- Xiang Sha Liu Jun Zi Tang - Adds aromatic Qi-moving herbs for fullness and poor appetite.
- Ling Gui Zhu Gan Tang - For fluid retention with dizziness from Spleen weakness.
- San Ren Tang - For Dampness with a muzzy head and heavy body.
Acupoints
- Zhongwan REN-12 - The Stomach front-mu point, used for fullness and nausea.
- Zusanli ST-36 - Strengthens the Stomach and Spleen, restoring normal digestion.
- Yinlingquan SP-9 - Drains Dampness from the middle and lower body.
- Pishu BL-20 - The Spleen back-shu point, supporting the source.
- Neiguan PC-6 - Stops nausea and settles the middle.
Personalising the plan
- If nausea dominates, aromatic and descending herbs are increased.
- If fullness dominates, Qi-moving herbs are increased.
- If the Spleen is weak, gentle support is added.
- If Heat appears, cooling herbs are added.
Self-care
Diet
- Warm light food; small portions
- Reduce dairy, sugar and fried food
- Avoid cold and raw food
- Barley and turnip are traditionally used for Dampness
Lifestyle
- Gentle walking after meals
- Do not lie down immediately after eating
- Avoid overeating
- Keep the environment dry
Qigong and movement
- Gentle practice emphasising the abdomen and slow breathing.
How it differs from look-alikes
Patterns that can be mistaken for phlegm-dampness in the middle-burner, and the contrast that separates them.
Spleen Qi Deficiency
Weakness without the thick greasy coat or the active obstruction.
Damp-Heat
A yellow greasy coat with thirst and heat; here it is white and cold.
Blood Stagnation
Fixed stabbing pain and a purple tongue rather than fullness and nausea.
Where it appears
Skin conditions in which this pattern is commonly seen.
Safety and when to get help
For anything severe, sudden or spreading quickly, read when a skin problem needs urgent care.
This page describes a TCM pattern, not a diagnosis, and a pattern can accompany more than one skin condition. Get urgent medical help for a rash with fever, confusion or feeling very unwell; skin that is blistering, peeling or spreading quickly; swelling of the face, lips or throat, or difficulty breathing; or a lesion with increasing pain, warmth, red streaks or pus. For anything persistent, severe or changing, have it assessed by a clinician before treating it yourself.
Read the skin condition pages for the red flags that belong to a specific presentation.
Urgent-sign guidance is based on standard sources: NHS anaphylaxis, NHS cellulitis, NHS Stevens-Johnson syndrome, NHS melanoma.
Evidence note
No controlled trial of this treatment in skin conditions was identified for this entry. What is described here rests on the classical and later materia medica record and on traditional clinical use, not on controlled trial evidence, and it is not a prediction of benefit. Where skin-specific research does exist it is linked above.
Common questions
Why is the coat the key sign?
A thick greasy coat shows Dampness and Phlegm directly. Its colour tells you whether Heat is present.
Why withhold tonics?
Rich tonics are dampening and hard to process. While the middle is congested they make the fullness worse, so clearing comes first.
Why does eating make it worse?
The middle is already obstructed, so more food adds to the congestion. Small warm meals are used precisely for this reason.
When should I see a doctor?
Persistent vomiting, weight loss, difficulty swallowing or blood in the stool needs medical assessment.
