Understanding Heart Fire blazing
The Heart is described as the seat of the spirit and as opening to the tongue. When Fire gathers there, it disturbs the spirit and shows on the tongue itself, which is why ulcers and a red tongue tip belong to this pattern.
Palpitations and restlessness are the inner signs. The mouth and tongue ulcers are the outer ones, and they are painful and recurrent. Sleep is light and full of dreams, or the person cannot settle at all.
The tongue is red with a distinctly redder tip, red points at the tip and a yellow dry coat. The pulse is rapid and may be full at the left cun position, which corresponds to the Heart.
It comes from prolonged emotional strain, external Heat that has moved inward, Liver Fire transmitting to the Heart, or overwork that has exhausted the mind.
Treatment clears Heart Fire, settles the spirit and, where necessary, drains the Heat downward. The strong bitter herbs are stopped once the Fire has cleared.

- Ulcers appear during periods of stress.
Practitioner notes
- The tongue is red with a distinctly redder tip, red points at the tip and a yellow dry coat.
- The pulse is rapid, and full at the left cun position which corresponds to the Heart.
- The distinction from Heart Yin deficiency is fullness: Fire is an excess with a rapid full pulse and a yellow coat, while Yin deficiency is empty and dry.
- The distinction from Phlegm-Fire harassing the Heart is the mental picture: that pattern has confusion and irrational behaviour, this one has agitation and insomnia.
Causes
Common causes
- Prolonged emotional stress and strain
- External Heat invading the interior
- Liver Fire transmitting to the Heart
- Overwork and mental exhaustion
How it develops
- It develops over weeks of strain or after a febrile illness.
- If it persists, it can damage Heart Yin and become a mixed picture.
Diagnosis
Diagnosis rests on the red tongue tip, rapid pulse and the combination of palpitations and mouth ulcers.
Tongue
- Body: Red with a distinctly redder tip and red points at the tip.
- Coating: Yellow and dry.
- Note: A pale tongue would rule this out entirely.
Pulse
Rapid, Full at the left cun
Fullness at the Heart position supports the diagnosis.
Observation
- Flushed face
- Restless manner
- Visible tongue or mouth ulcers
Listening
- Fast, agitated speech
Palpation
- Abdomen: Unremarkable unless another pattern is present.
- Channels: HT-7 and PC-8 may be tender; the chest may feel warm.
Questions that separate it from look-alikes
- Any mouth or tongue ulcers?
- How is sleep, and is it restless?
- Any palpitations?
- How much stress, anger and spicy food lately?
Treatment
Principles
- Clear Heart Fire
- Settle the spirit
- Drain the Heat downward where it has accumulated
- Cool the Blood where ulcers are marked
Herbs
Formulas
- Dao Chi San - The reference formula for Heart Fire, draining it through the urine.
- Xie Xin Tang - For Fire in the Heart with a burning sensation and agitation.
- Huang Lian Jie Du Tang - For Heat with toxin, used where the Fire is generalised.
- Zhu Sha An Shen Wan - For restlessness and insomnia from Heart Fire, used short term.
- Qing Xin Lian Zi Yin - For Heat in the Heart with restlessness and dry mouth.
Acupoints
- HT-5 (Tongli) - Clears Heat from the Heart and calms the spirit, used for palpitations and ulcers.
- PC-8 (Laogong) - Clears Heart Fire strongly, used for agitation and mouth ulcers.
Personalising the plan
- If ulcers dominate, Blood-cooling herbs are added.
- If constipation is present, draining herbs are added.
- If the spirit is very agitated, calming herbs are increased.
- If Yin is being damaged, nourishing herbs are added.
Self-care
Diet
- Cooling food: watermelon, cucumber, mung bean, lotus seed
- Avoid spicy, fried and rich food
- Reduce alcohol and coffee
Lifestyle
- A calm environment and adequate sleep
- Reduce mental strain
- Gentle exercise
- Manage anger actively
Qigong and movement
- Slow breathing and quiet practice, done in the evening to settle the spirit.
How it differs from look-alikes
Patterns that can be mistaken for heart fire blazing, and the contrast that separates them.
Heart Yin Deficiency
A dry, empty picture with night sweats and a fine pulse. Fire is full, with a rapid full pulse and a yellow coat.
Liver Fire Blazing
Liver Fire gives red eyes, bitter taste and temple headache; here the tongue tip and mouth ulcers point to the Heart.
Phlegm-Fire harassing the Heart
That pattern has confusion and irrational behaviour with a greasy coat; this one has agitation and insomnia.
Where it appears
Skin conditions in which this pattern is commonly seen.
Safety and when to get help
For anything severe, sudden or spreading quickly, read when a skin problem needs urgent care.
This page describes a TCM pattern, not a diagnosis, and a pattern can accompany more than one skin condition. Get urgent medical help for a rash with fever, confusion or feeling very unwell; skin that is blistering, peeling or spreading quickly; swelling of the face, lips or throat, or difficulty breathing; or a lesion with increasing pain, warmth, red streaks or pus. For anything persistent, severe or changing, have it assessed by a clinician before treating it yourself.
Read the skin condition pages for the red flags that belong to a specific presentation.
Urgent-sign guidance is based on standard sources: NHS anaphylaxis, NHS cellulitis, NHS Stevens-Johnson syndrome, NHS melanoma.
Evidence note
No controlled trial of this treatment in skin conditions was identified for this entry. What is described here rests on the classical and later materia medica record and on traditional clinical use, not on controlled trial evidence, and it is not a prediction of benefit. Where skin-specific research does exist it is linked above.
Common questions
Why do the ulcers appear on the tongue?
The Heart opens to the tongue, so Heart Fire shows there directly. It is one of the clearest examples of an internal pattern appearing on a visible surface.
Why is sleep affected?
The Heart houses the spirit. When Fire disturbs the Heart, the spirit cannot settle, so sleep is light and full of dreams.
Why drain the heat downward?
Because Fire that is cleared only at the top can remain in the body. Draining it through the urine or bowels removes it.
When should I see a doctor?
Palpitations with chest pain, fainting, or ulcers that do not heal needs medical assessment.
