Understanding Phlegm-Heat

Phlegm becomes Heat when it sits long enough or when Heat is added. The result is thicker, yellower phlegm and a more irritable picture, and because Phlegm can obstruct the mind as well as the chest, restlessness is part of it.

The cough produces thick sticky yellow phlegm that is hard to bring up. The chest feels full or oppressed, and there may be breathlessness or wheeze.

Restlessness and irritability accompany it, and sleep is disturbed. The tongue is red and swollen with a thick yellow greasy coat, and the pulse is slippery and rapid.

It comes from a Wind-Heat or Wind-Cold invasion that transformed into interior Heat, from greasy rich food and alcohol, from emotional stress generating both Phlegm and Heat, or from long-standing Damp-Phlegm that has heated.

Treatment clears Heat and transforms Phlegm together. Doing only one leaves the other: clearing alone leaves the Phlegm, transforming alone leaves the Heat to thicken it again.

Signs and symptoms

Key signs

  • Coughing up thick sticky yellow phlegm
  • Fullness or oppression in the chest
  • Restlessness or irritability
  • Yellow greasy tongue coating with a slippery rapid pulse

Also seen

  • Breathlessness
  • Wheeze
  • A bitter taste
  • Thirst
  • Disturbed sleep
  • A flushed face

Variations

  • Phlegm that is difficult to expectorate
  • A rattling chest
  • Night-time worsening of the cough

Worse with

  • Greasy or fried food
  • Alcohol
  • Smoking

Better with

  • Cool air
  • Coughing up phlegm
  • Light cooling food
  • Rest

Timing

  • The cough is worse at night and on waking.
  • It follows a cold that has lingered and heated.

Practitioner notes

  • The tongue is red and swollen with a thick yellow greasy coat.
  • The pulse is slippery and rapid: slippery shows Phlegm, rapid shows Heat.
  • The distinction from Damp-Phlegm is colour and heat: that is white phlegm with a pale tongue and no Heat.
  • The distinction from Lung Heat is the amount of phlegm: here Phlegm is abundant and obstructing, while Lung Heat has thick phlegm without the greasy coat.

Causes

Common causes

  • Wind-Heat or Wind-Cold transforming into interior Heat
  • Greasy, sweet or rich food and alcohol
  • Emotional stress generating Qi stagnation, Phlegm and Heat
  • Spleen weakness allowing Phlegm to accumulate and heat

How it develops

  • It develops from unresolved phlegm that has heated.
  • If the Heat deepens, it can affect the mind, which is a more serious picture.

Diagnosis

Diagnosis rests on thick yellow phlegm with restlessness and a yellow greasy coat.

Tongue

  • Body: Red and swollen.
  • Coating: Thick yellow greasy.
  • Note: A white greasy coat would indicate Damp-Phlegm instead.

Pulse

Slippery, Rapid

Slippery shows Phlegm, rapid shows Heat.

Observation

  • Flushed face
  • Restlessness
  • Audible chest congestion

Listening

  • A loose or rattling cough with thick sputum

Palpation

  • Abdomen: Possible fullness in the upper abdomen.
  • Channels: The chest may feel warm; ST-40 and BL-13 may be sensitive.

Questions that separate it from look-alikes

  • What colour is the phlegm, and is it easy to bring up?
  • Is the chest tight or full?
  • Any restlessness or disturbed sleep?
  • How much alcohol, fried food and smoking?

Treatment

Principles

  • Clear Heat
  • Transform thick Phlegm
  • Open the chest and redirect Qi downward
  • Settle the spirit where restlessness is present

Herbs

Formulas

  • Qing Qi Hua Tan Wan - The reference formula for Phlegm-Heat with thick yellow phlegm, clearing and transforming.
  • Wen Dan Tang - For Phlegm-Heat with restlessness, dizziness and nausea, settling the mind as well.
  • Xiao Xian Xiong Tang - For Phlegm-Heat with chest oppression and difficulty expectorating.
  • Ding Chuan Tang - For Phlegm-Heat with wheeze and breathlessness.

Acupoints

  • Fenglong ST-40 - The principal point for transforming Phlegm.
  • Lieque LU-7 - Opens the Lungs and redirects their Qi downward.
  • Quchi LI-11 - Clears Heat from the body.
  • Zhongwan REN-12 - Transforms Dampness and Phlegm in the middle.
  • Feishu BL-13 - The Lung back-shu point, used for cough and congestion.
  • Neiguan PC-6 - Opens the chest and settles the spirit.

Personalising the plan

  • If Heat dominates, cooling herbs are increased.
  • If Phlegm dominates, transforming herbs are increased.
  • If the chest is tight, herbs that open it are added.
  • If restlessness dominates, spirit-settling herbs are added.

Self-care

Diet

  • Light cooling food; reduce dairy, sugar and fried food
  • Stop alcohol and smoking
  • Barley, pear and turnip are traditionally used for Phlegm and Heat

Lifestyle

  • Rest and avoid smoky environments
  • Cool well-ventilated room
  • Adequate water
  • Avoid heavy meals late at night

Qigong and movement

  • Gentle breathing practice; avoid exertion during flares.

How it differs from look-alikes

Patterns that can be mistaken for phlegm-heat, and the contrast that separates them.

Damp-Phlegm

White plentiful phlegm with a pale wet tongue and no heat signs; this pattern is yellow, thick and hot.

Lung Heat

Heat without the thick greasy coat and restlessness; here Phlegm is a prominent part of the picture.

Liver Fire Blazing

Red eyes, bitter taste and temple headache rather than productive cough and chest oppression.

Cold-Phlegm

Clear thin phlegm with coldness and a white coat; this pattern is the hot counterpart.

Safety and when to get help

For anything severe, sudden or spreading quickly, read when a skin problem needs urgent care.

This page describes a TCM pattern, not a diagnosis, and a pattern can accompany more than one skin condition. Get urgent medical help for a rash with fever, confusion or feeling very unwell; skin that is blistering, peeling or spreading quickly; swelling of the face, lips or throat, or difficulty breathing; or a lesion with increasing pain, warmth, red streaks or pus. For anything persistent, severe or changing, have it assessed by a clinician before treating it yourself.

Read the skin condition pages for the red flags that belong to a specific presentation.

Urgent-sign guidance is based on standard sources: NHS anaphylaxis, NHS cellulitis, NHS Stevens-Johnson syndrome, NHS melanoma.

Evidence note

No controlled trial of this treatment in skin conditions was identified for this entry. What is described here rests on the classical and later materia medica record and on traditional clinical use, not on controlled trial evidence, and it is not a prediction of benefit. Where skin-specific research does exist it is linked above.

Common questions

Why is the phlegm so hard to bring up?

Heat thickens phlegm and dries the fluid in it, so it becomes sticky. The herbs that transform it also loosen it.

Why is restlessness part of a chest pattern?

Phlegm can obstruct the mind as well as the Lungs. When Heat is added the person becomes irritable and sleep is disturbed.

Why treat Heat and Phlegm together?

Clearing only the Heat leaves the Phlegm, and transforming only the Phlegm leaves the Heat to thicken it again.

When should I see a doctor?

Breathlessness, chest pain, high fever or coughing blood needs medical assessment.
