Understanding Heat and Blood Stagnation in the Lower Burner
This pattern describes Blood that has pooled in the lower abdomen together with Heat. Because the stagnation is fixed and the Heat has nowhere to disperse, the disturbance can affect the mind as well as the abdomen.
The pain is fixed and sharp low in the abdomen and is worse with pressure. The area may feel hard or full. The blood, whether menstrual or in the stool, is dark.
Restlessness, agitation or behaviour that seems out of character is described as part of the picture, and it resolves as the stagnation clears.
The tongue is reddish-purple with stasis spots and a dry yellow coat. The pulse is deep, choppy and often rapid.
It comes from suppressed anger and stress, hot spicy food and alcohol, trauma or surgery, or prolonged sitting.
Treatment breaks the stasis and drains the Heat. Because the pattern can involve acute abdominal signs, medical assessment is needed to exclude conditions that require surgery.
Signs and symptoms
Key signs
- Fixed sharp pain in the lower abdomen that worsens with pressure
- Lower abdominal hardness or fullness
- Dark-coloured blood
- Mental restlessness or unusual behaviour
Also seen
- Pain that is worse at night
- A dry mouth with little thirst
- Dark stools or dark menstrual blood
- Restlessness that is hard to explain
- A feeling of heat in the lower abdomen
Variations
- Amnesia or confusion in severe classical cases
- Pain radiating to the lower back
Worse with
- Night-time
- Pressure on the lower abdomen
- Emotional stress or anger
Better with
- Passage of dark blood or stool
- A bowel movement
- Cooling the lower abdomen
Timing
- Pain and fever are worse at night.
- The picture builds over days rather than minutes.
Practitioner notes
- The tongue is reddish-purple with stasis spots and a dry yellow coat.
- The pulse is deep, choppy and rapid, showing stasis with Heat in the interior.
- The distinction from Liver Qi stagnation is fixity and hardness: here the abdomen is fixed and tender.
- The distinction from Damp-Heat is the blood: here there is dark pooled blood rather than discharge or a greasy coat.
Causes
Common causes
- Suppressed anger and emotional stress
- Hot, spicy, greasy food and alcohol
- Physical trauma, surgery or difficult childbirth
- Prolonged sitting and lack of movement
How it develops
- It develops over days from stasis that has become fixed.
- Acute abdominal signs are a medical matter and override pattern reasoning.
Diagnosis
Diagnosis rests on fixed hard lower abdominal pain with dark blood and signs of Heat, and is made alongside medical assessment.
Tongue
- Body: Reddish-purple with stasis spots and distended sublingual veins.
- Coating: Dry and yellow.
- Note: A pale tongue would suggest a different pattern.
Pulse
Deep, Choppy, Rapid
Deep and choppy shows interior stasis; rapid shows Heat.
Observation
- Restlessness
- Guarding of the lower abdomen
- A flushed face
Listening
- Agitated or unusual speech in severe cases
Palpation
- Abdomen: Fixed tenderness and hardness low in the abdomen, worse on pressure. This is a finding that requires medical assessment.
- Channels: SP-6 and REN-4 may be tender.
Questions that separate it from look-alikes
- Where exactly is the pain, and is it fixed?
- Is there hardness or fullness?
- Is there dark blood in the stool or the period?
- Has this been medically assessed?
Treatment
Principles
- Medical assessment of acute abdominal signs first
- Break Blood stasis
- Drain the Heat
- Settle the spirit where the stagnation has disturbed it
Herbs
Formulas
- Tao He Cheng Qi Tang - The reference formula for stasis with Heat and fullness in the lower abdomen.
- Da Huang Mu Dan Pi Tang - For stasis with Heat and a fixed mass-like fullness.
- Gui Zhi Fu Ling Wan - For stasis in the lower abdomen, gentler than the draining formulas.
- Di Dang Tang - A strong formula for stubborn stasis, used only under professional supervision.
- Xia Yu Xue Tang - For Blood stasis in the lower burner with pain.
Acupoints
- REN-4 (Guanyuan) - Regulates the lower abdomen and is used for stasis and pain.
- SP-6 (Sanyinjiao) - Moves Blood and regulates the lower body.
- LI-4 (Hegu) - Moves Qi and Blood, used for abdominal pain.
- LR-8 (Ququan) - Regulates the Liver channel in the lower body.
- BL-17 (Geshu) - The influential point for Blood.
Personalising the plan
- If the bowels are constipated, draining herbs are increased under supervision.
- If the pain is severe, the stasis-breaking herbs are increased with care.
- If agitation dominates, spirit-settling herbs are added.
- If the person is depleted, strong draining is reduced so they are not exhausted.
Self-care
Diet
- Light cooling food; avoid hot, spicy and greasy food
- Stop alcohol
- Adequate water
Lifestyle
- Seek medical assessment for severe or fixed abdominal pain
- Gentle movement rather than prolonged sitting
- Manage anger and stress
- Do not apply heat if the abdomen is hot and tender
Qigong and movement
- Gentle movement between episodes only, with medical clearance.
How it differs from look-alikes
Patterns that can be mistaken for heat and blood stagnation in the lower burner, and the contrast that separates them.
Liver Qi Stagnation
Distending, moving pain without hardness or dark blood; the abdomen is not fixed or tender.
Damp-Heat
A greasy coat, discharge and heaviness rather than pooled dark blood and hardness.
Where it appears
Skin conditions in which this pattern is commonly seen.
Safety and when to get help
For anything severe, sudden or spreading quickly, read when a skin problem needs urgent care.
This page describes a TCM pattern, not a diagnosis, and a pattern can accompany more than one skin condition. Get urgent medical help for a rash with fever, confusion or feeling very unwell; skin that is blistering, peeling or spreading quickly; swelling of the face, lips or throat, or difficulty breathing; or a lesion with increasing pain, warmth, red streaks or pus. For anything persistent, severe or changing, have it assessed by a clinician before treating it yourself.
Read the skin condition pages for the red flags that belong to a specific presentation.
Urgent-sign guidance is based on standard sources: NHS anaphylaxis, NHS cellulitis, NHS Stevens-Johnson syndrome, NHS melanoma.
Evidence note
No controlled trial of this treatment in skin conditions was identified for this entry. What is described here rests on the classical and later materia medica record and on traditional clinical use, not on controlled trial evidence, and it is not a prediction of benefit. Where skin-specific research does exist it is linked above.
Common questions
Why is there mental restlessness?
The classical texts describe stagnation in the lower abdomen as disturbing the spirit. It resolves as the stagnation clears, but any confusion should be medically assessed.
Why is medical assessment needed first?
A fixed, hard, tender abdomen can indicate conditions that need surgery. Pattern treatment is not a substitute for that assessment.
Why include a warming herb among the cooling ones?
Gui Zhi keeps the Blood moving. Without movement, cooling alone does not resolve stasis.
When is it urgent?
Sudden severe abdominal pain, fever, a rigid abdomen or vomiting needs emergency assessment.
