What You Gui Wan is for
The skin this pill is for has gone cold: pale, thin and slow to renew, with dusky extremities and a surface that heals and regrows slowly. Yang and Essence are both low, so the body cannot warm the surface or supply the material for repair, and the skin looks older and more depleted than the person's years. The formula warms the Yang and fills the Essence, which is why it works over months rather than on a single lesion.
The colour is the first clue: pale, with a grey or dusky cast at the fingers, toes and lips, and a surface that is cool to the touch even in a warm room. The skin is thin and dry, heals slowly from small injuries and marks easily, and hair and nails grow slowly and look thin. There is no redness and no heat, and the surface does not thicken or scale. It is a quiet, cold, undernourished picture, and it has usually been present for a long time.
Zhi Fu Zi and Rou Gui warm the Yang at the root, which is what reaches the surface as warmth and colour. Lu Jiao Jiao is the strongest Essence tonic in the formula and supplies the material for renewal of tissue, hair and bone, while Shu Di Huang, Shan Zhu Yu, Shan Yao, Gou Qi Zi, Tu Si Zi and Dang Gui rebuild the Essence and Blood that give the skin its substance. It is rich and warming throughout, so nothing in it is cooling or drying.
It is used for the deep cold depleted picture, over weeks, and it is not for skin that is red, hot or dry from Yin deficiency, where the warming would make matters worse. It contains Fu Zi, which is toxic unless it is correctly prepared and dosed, so it is prescribed and supervised rather than taken independently, and it is contraindicated in pregnancy. Cold extremities that come with chest pain or breathlessness need urgent assessment rather than a warming course.
Actions
- Warms Kidney Yang
- Tonifies the Essence
- Nourishes Blood
- Strengthens the lower back and legs
Properties
- Taste and temperature: Predominantly sweet and pungent with a hot quality. Sweet tonifies the Essence and Blood, hot warms the Yang.
- Organs: Kidney, Liver, Spleen
- Channels: Kidney, Liver
The pattern behind it
The pattern is Kidney Yang and Essence deficiency. The Yang warms and drives, the Essence underlies growth and repair, and when both are low the body is cold, weak and slow to renew its tissue. The formula warms the Yang and fills the Essence, which is why it is used for the cold depleted picture rather than for heat or dryness.

Patterns it treats
Kidney Yang Deficiency
Cold, pale, thin skin with a dusky cast at the fingers, toes and lips, slow to heal and slow to renew hair and nails. Yang and Essence are both low, so the surface is neither warmed nor rebuilt and looks older and more depleted than the person's years.
How the formula is built
The formula warms and fills at the same time. Fu Zi and Rou Gui warm the Yang, Lu Jiao Jiao and the nourishing group fill the Essence and Blood, and Dang Gui keeps it moving. That combination is why it is used for deep cold depletion rather than for a single deficiency, and why it requires supervision.

Origin and history
Jing Yue Quan Shu (景岳全书, The Complete Works of Jingyue) by Zhang Jingyue, Ming dynasty, 17th century.
Zhang Jingyue created Zuo Gui Wan and You Gui Wan as a pair, named after the left and right Kidneys of the classical model: one to enrich the Yin and one to warm the Yang. You Gui Wan became the standard warming Essence tonic, used for the deep cold depletion that shows on the skin as pallor, coldness and slow renewal.
Classical record
Zhang Jingyue recorded the pill as the warming counterpart to his Yin tonic, for Kidney Yang and Essence deficiency with cold limbs, lower back weakness and a weak pulse.

Evidence note
No controlled trial of this treatment in skin conditions was identified for this entry. What is described here rests on the classical and later materia medica record and on traditional clinical use, not on controlled trial evidence, and it is not a prediction of benefit. Where skin-specific research does exist it is linked above.
Common questions
How is it different from Zuo Gui Wan?
Zuo Gui Wan nourishes Yin and Essence. You Gui Wan warms Yang and fills the Essence, so it is used for the cold patient with pale skin and cold limbs.

When should cold limbs be investigated?
Cold limbs with chest pain, breathlessness, colour change or one-sided symptoms needs urgent medical assessment.
