Understanding Excess-Heat
The tradition distinguishes excess Heat from empty Heat by fullness. Excess heat is strong and forceful: the pulse pushes back, the face is red, and the person wants cold. Empty heat is weak and empty, coming from deficiency.
The heat is high and constant, with intense thirst that only cold drinks satisfy. The face is red, the person is restless, and the pulse is rapid and forceful.
The tongue is red, possibly with raised red points, with a thick dry yellow coat. The urine is dark and scanty and the stools may be dry.
It arises from external Heat or Summer Heat, from Cold that has transformed into Heat inside the body, from intense emotion, or from overwork and sleep deprivation.
Treatment clears and drains the Heat. Strong bitter-cold herbs are used, and they are stopped once the Heat is cleared because prolonged use damages the digestion.
Signs and symptoms

- Red face
- Rapid and forceful pulse
Also seen
- Restlessness and irritability
- Dark scanty urine
- Dry stools or constipation
- Sweating that does not relieve the heat
- A bitter taste
- Red eyes
Variations
- Mouth ulcers
- Nosebleeds
- A throbbing headache
Worse with
- Hot weather or warm environments
- Spicy, greasy or fried food
- Alcohol
Better with
- Cool environments
- Cold drinks and cool food
- Rest
Timing
- It is acute and constant rather than intermittent.
- It worsens in the afternoon and in heat.
Practitioner notes
- The tongue is red, possibly with raised red points, with a thick dry yellow coat.
- The pulse is rapid and forceful: the force is what separates excess from empty heat.
- The distinction from empty heat is fullness: empty heat has a fine rapid pulse and a dry tongue with no coat.
- The distinction from Bright Yang Stomach Heat is specificity: that pattern localises to the Stomach with the four classic signs.
Causes
Common causes
- Invasion by external Heat or Summer Heat
- Cold pathogens transforming into Heat inside the body
- Prolonged anger, frustration or stress
- Overwork and sleep deprivation
How it develops
- It develops acutely or from an unresolved earlier pattern.
- If untreated, it can damage fluids and Yin, becoming a mixed deficiency and excess picture.
Diagnosis
Diagnosis rests on strong heat signs with a forceful pulse and a thick yellow coat.
Tongue
- Body: Red, possibly with raised red points.
- Coating: Thick, dry and yellow.
- Note: A tongue with no coat would suggest empty heat instead.
Pulse
Rapid, Forceful, Slippery
Force is the key: it shows the heat is excess.
Observation
- Red face
- Restlessness
- Sweating
Listening
- A loud or sharp voice
Palpation
- Abdomen: May feel hot; fullness if the bowels are affected.
- Channels: The skin is hot; DU-14 and LI-11 may feel hot.
Questions that separate it from look-alikes
- Is there thirst, and for cold drinks?
- How high is the fever?
- Any constipation or dark urine?
- How much alcohol, spicy food and stress lately?
Treatment
Principles
- Clear Heat
- Drain Fire where it is localised
- Protect the fluids
- Use strong clearing briefly rather than long term
Herbs
Formulas
- Bai Hu Tang - The reference formula for excess Heat with fever, sweating and thirst.
- Huang Lian Jie Du Tang - For Heat with toxin, draining Fire from the whole body.
- Qing Ying Tang - For Heat that has entered the nutritive level, with fever at night.
- Long Dan Xie Gan Tang - For excess Heat localised in the Liver and Gallbladder.
- Liang Ge San - For Heat in the upper body with sore throat and mouth symptoms.
Acupoints
- LI-4 (Hegu) - Clears Heat from the upper body.
- DU-14 (Dazhui) - Clears Heat from the whole body, used in febrile conditions.
- ST-44 (Neiting) - Clears Heat from the Stomach and the face.
- SP-10 (Xuehai) - Cools the Blood, used where Heat has entered it.
- LR-2 (Xingjian) - Drains Liver Fire, used for red eyes and irritability.
Personalising the plan
- If the bowels are constipated, draining herbs are added.
- If the fluids are damaged, moistening herbs are increased.
- If the Heat is localised, herbs specific to that area are used.
- If the person is depleted, the strong clearing is reduced and support added.
Self-care
Diet
- Cooling food and drink: water, mung bean, watermelon, cucumber
- Avoid spicy, fried and greasy food
- Stop alcohol while the heat persists
Lifestyle
- Rest in a cool environment
- Light clothing
- Sleep early
- Reduce stress and anger
Qigong and movement
- Gentle practice in a cool setting, avoiding exertion that raises body heat.
How it differs from look-alikes
Patterns that can be mistaken for excess-heat, and the contrast that separates them.
Empty-Heat caused by Yin Deficiency
Weak and dry, with a fine rapid pulse and no coat. Excess heat is forceful with a thick yellow coat.
Damp-Heat
Adds Dampness: a greasy coat, heaviness and sticky stools, rather than the dry heat of this pattern.
Liver Fire Blazing
Localised to the Liver with red eyes, bitter taste and temple headache.
Bright Yang Stomach Heat
A specific form of excess heat in the Stomach with the four classic signs: fever, sweat, thirst and surging pulse.
Where it appears
Skin conditions in which this pattern is commonly seen.
Safety and when to get help
For anything severe, sudden or spreading quickly, read when a skin problem needs urgent care.
This page describes a TCM pattern, not a diagnosis, and a pattern can accompany more than one skin condition. Get urgent medical help for a rash with fever, confusion or feeling very unwell; skin that is blistering, peeling or spreading quickly; swelling of the face, lips or throat, or difficulty breathing; or a lesion with increasing pain, warmth, red streaks or pus. For anything persistent, severe or changing, have it assessed by a clinician before treating it yourself.
Read the skin condition pages for the red flags that belong to a specific presentation.
Urgent-sign guidance is based on standard sources: NHS anaphylaxis, NHS cellulitis, NHS Stevens-Johnson syndrome, NHS melanoma.
Evidence note
No controlled trial of this treatment in skin conditions was identified for this entry. What is described here rests on the classical and later materia medica record and on traditional clinical use, not on controlled trial evidence, and it is not a prediction of benefit. Where skin-specific research does exist it is linked above.
Common questions
How is excess heat different from empty heat?
By force. The pulse is forceful here and fine in empty heat, and the tongue has a thick yellow coat here rather than being dry with no coat.
Why are the strong herbs used briefly?
They are extremely bitter and cold. Over time they damage digestion and deplete Yin, so they are stopped as the heat clears.
Why protect the fluids?
Heat dries the body, and strong clearing can add to that. Pairing the cooling with moistening prevents the treatment from causing new dryness.
When should I see a doctor?
High fever, confusion, breathlessness or an inability to keep fluids down needs medical assessment.
