Understanding Kidney and Liver Yin Deficiency causing Heat in the Blood
When Yin is deficient for long enough, the Heat that results is said to enter the Blood. Because Blood is the deeper layer, the signs become bleeding and flushing rather than just dryness.
Night sweats and afternoon flushing come with a dry mouth and throat, worse at night. The lower back and knees are weak and sore.
Bleeding appears in various forms: nosebleeds, heavy periods, blood in the urine, or small purpuric spots on the skin. The blood is usually bright or dark red rather than pale.
The tongue is red and thin with little or no coating, possible cracks and red spots, and red sides. The pulse is fine and rapid.
The causes include prolonged frustration and anger, chronic overwork, multiple pregnancies and breastfeeding, and menopause. Treatment nourishes the Yin and cools the Blood, and any significant or unexplained bleeding is medically assessed.
Signs and symptoms
Key signs
- Night sweats or afternoon tidal flushing
- Bleeding tendencies such as nosebleeds, heavy periods or purpura
- Low back and knee weakness
- Dry mouth and throat, especially at night
Also seen
- Five-palm heat
- Tinnitus
- Dry eyes
- Irritability
- Scanty dark urine
- Poor sleep
Variations
- Bleeding gums
- Blood-streaked sputum
- Hot flushes at menopause
Worse with
- Staying up late or sleep deprivation
- Emotional stress, anger or frustration
- Overwork or excessive mental strain
Better with
- Adequate rest
- Cooling moistening food
- Calm environment
- Gentle exercise
Timing
- The flushing and sweats are worse in the afternoon and at night.
- Bleeding may follow a cyclical pattern in women.
Practitioner notes
- The tongue is red and thin, dry with little or no coating, with possible cracks, red spots and red sides.
- The pulse is fine and rapid.
- The distinction from plain Heat in the Blood is the deficiency: here the pulse is fine and there is lower back weakness and dryness.
- The distinction from Liver Fire is emptiness: Fire is forceful with red eyes and a bitter taste, without the deep Yin depletion.
Causes
Common causes
- Prolonged frustration and anger
- Chronic overwork and insufficient rest
- Multiple pregnancies and breastfeeding
- Ageing and menopause
How it develops
- It develops from long-standing Yin deficiency that has heated.
- If the bleeding continues, it deepens the deficiency, which worsens the Heat.
Diagnosis
Diagnosis rests on Yin deficiency signs together with bleeding or flushing and a red dry tongue.
Tongue
- Body: Red and thin, dry, with possible cracks, red spots and red sides.
- Coating: Little or none.
- Note: A greasy coat would point to Damp-Heat instead.
Pulse
Fine, Rapid
Fine shows deficiency; rapid shows the Heat.
Observation
- Flushing over the cheekbones
- Dry lips
- Possible purpuric spots
Listening
- Reports of night sweats
- Irritability in the voice
Palpation
- Abdomen: Soft and empty below the navel.
- Channels: KI-3 and LR-3 may feel empty; the skin may be dry and warm.
Questions that separate it from look-alikes
- Any bleeding, and where?
- How are the periods?
- Any night sweats, flushing or dry mouth at night?
- How much overwork and how is sleep?
Treatment
Principles
- Nourish Kidney and Liver Yin
- Cool the Blood and stop the bleeding
- Clear the empty heat gently
- Assess any significant bleeding medically
Herbs
Formulas
- Liang Di Tang - The reference formula for this pattern: cooling the Blood while nourishing Yin.
- Qing Jing San - For heat in the Blood with flushing and bleeding.
- Zhi Bai Di Huang Wan - For Kidney Yin deficiency with empty heat.
- Er Zhi Wan - A two-herb Yin tonic, used for early Yin deficiency with heat.
- Da Bu Yin Wan - A deep Yin tonic for advanced depletion with heat.
- Yi Guan Jian - For Liver and Kidney Yin deficiency with dryness.
Acupoints
- Zhaohai KI-6 - Nourishes Kidney Yin and is used for night sweats.
- Taichong LR-3 - The Liver source point, used for the Liver Yin and anger component.
- Xuehai SP-10 - Cools the Blood and is used for bleeding and heavy periods.
- Guanyuan REN-4 - Strengthens the deeper Yin reserves.
Personalising the plan
- If bleeding dominates, hemostatic herbs are increased.
- If flushing and sweats dominate, heat-clearing herbs are increased.
- If the lower back weakness dominates, Kidney tonics are increased.
- If the person is depleted, the cooling is kept gentle so it does not drain them.
Self-care
Diet
- Cooling moistening food: pear, black sesame, goji, mung bean
- Avoid spicy, fried and drying food
- Reduce alcohol and coffee
Lifestyle
- Avoid late nights; sleep is the main replenishment
- Reduce overwork and emotional strain
- Gentle exercise
- Report any unusual or heavy bleeding to a doctor
Qigong and movement
- Slow restorative practice, avoiding heat-generating exertion.
How it differs from look-alikes
Patterns that can be mistaken for kidney and liver yin deficiency causing heat in the blood, and the contrast that separates them.
Kidney and Liver Yin Deficiency
The deficiency without the active bleeding and flushing; here the Heat has reached the Blood.
Heat in the Blood
A broader and often fuller pattern; here the deficiency and the deep dryness are clear.
Liver Fire Blazing
A forceful pulse with red eyes and bitter taste, without the deep Yin depletion.
Kidney Yin Deficiency With Empty-Heat Blazing
A close pattern; here the bleeding tendency and the Liver involvement are more prominent.
Where it appears
Skin conditions in which this pattern is commonly seen.
Safety and when to get help
For anything severe, sudden or spreading quickly, read when a skin problem needs urgent care.
This page describes a TCM pattern, not a diagnosis, and a pattern can accompany more than one skin condition. Get urgent medical help for a rash with fever, confusion or feeling very unwell; skin that is blistering, peeling or spreading quickly; swelling of the face, lips or throat, or difficulty breathing; or a lesion with increasing pain, warmth, red streaks or pus. For anything persistent, severe or changing, have it assessed by a clinician before treating it yourself.
Read the skin condition pages for the red flags that belong to a specific presentation.
Urgent-sign guidance is based on standard sources: NHS anaphylaxis, NHS cellulitis, NHS Stevens-Johnson syndrome, NHS melanoma.
Evidence note
No controlled trial of this treatment in skin conditions was identified for this entry. What is described here rests on the classical and later materia medica record and on traditional clinical use, not on controlled trial evidence, and it is not a prediction of benefit. Where skin-specific research does exist it is linked above.
Common questions
Why does Yin deficiency cause bleeding?
Prolonged deficiency produces empty heat, and heat is said to drive the Blood out of its vessels. Because the deficiency is deep, the bleeding is usually chronic and low-grade.
Why is the flooding worse in the afternoon?
Empty heat is described as rising at the time when Yin is most depleted. Afternoon flushing is characteristic of this kind of heat.
Why treat the bleeding medically?
Bleeding has many causes and some are serious. Pattern treatment is supportive and follows assessment, not instead of it.
When should I see a doctor?
Any unexplained bleeding, heavy periods, blood in the urine or new purpura needs medical assessment.
