Understanding Heat in the Ying Level
Warm-disease theory describes four levels through which Heat passes: Wei, Qi, Ying and Blood. Ying is the nutritive level, closer to the interior, where Heat begins to affect the Blood and the spirit.
The fever is worse at night, and there is restlessness or insomnia rather than the clouded consciousness of the Pericardium stage. The person may be agitated but remains oriented.
The tongue is deep crimson and dry with little or no coat and possible red prickles at the tip. The pulse is fine and rapid, reflecting Heat that has damaged the fluids.
It progresses from unresolved Qi-level Heat, or directly from the surface, or arises more easily where Yin is already deficient.
Treatment clears the Ying level and cools the Blood, and it must be done promptly because the next stage involves the Pericardium. Any high fever with an altered mental state needs medical assessment.
Signs and symptoms
Key signs
- Fever that worsens at night
- Deep crimson tongue with little or no coating
- Mental restlessness or insomnia
- Fine and rapid pulse
Also seen
- Thirst with little desire to drink
- A dry mouth
- Restlessness that is hard to settle
- A faint rash of small red points
- Dark scanty urine
Variations
- Agitation without confusion
- A feeling of heat in the chest
- Faint red spots on the skin
Worse with
- Night-time
- Hot environments
- Spicy or warming food
Better with
- Cooling measures
- Rest in a cool quiet place
- Medical treatment of the underlying cause
Timing
- The fever is worse at night, which is characteristic of this level.
- It develops over days in a febrile illness.
Practitioner notes
- The tongue is deep crimson and dry with little or no coat, sometimes with red prickles at the tip.
- The pulse is fine and rapid: fine shows the fluid damage, rapid shows the Heat.
- The distinction from Qi-level Heat is the tongue: the Qi level has a yellow coat, the Ying level has almost no coat and a crimson body.
- The distinction from Heat in the Pericardium is consciousness: this level keeps the mind oriented, the next loses it.
Causes
Common causes
- Progression from unresolved Qi-level Heat
- Direct transmission from the surface
- Pre-existing Yin deficiency making the body vulnerable
- Epidemic Heat-toxins
How it develops
- It develops over days from a febrile illness.
- If not cleared, it progresses to Heat in the Pericardium or to the Blood level.
Diagnosis
Diagnosis rests on the crimson dry tongue, night fever and restlessness with an intact mind.
Tongue
- Body: Deep crimson and dry, sometimes with red prickles at the tip.
- Coating: Little or none.
- Note: A yellow coat would suggest the Heat is still at the Qi level.
Pulse
Fine, Rapid
Fine and rapid shows Heat with fluid damage.
Observation
- Flushed face
- Restless manner
- Dry lips
Listening
- Fast speech
- Reports of poor sleep
Palpation
- Abdomen: May be warm.
- Channels: The skin is hot and dry; the chest may feel warm to the touch.
Questions that separate it from look-alikes
- Is the fever worse at night?
- How is sleep and the mind?
- Any thirst, and is drinking wanted?
- How long has the fever lasted?
Treatment
Principles
- Clear Heat from the Ying level
- Cool the Blood
- Nourish the fluids being consumed
- Settle the spirit
Herbs
Formulas
- Qing Ying Tang - The reference formula for this exact level: it clears the Ying and cools the Blood.
- Qing Gong Tang - For Heat moving toward the Pericardium, opening the orifices.
- An Gong Niu Huang Wan - For the next stage with clouded consciousness; hospital use only.
Acupoints
- Ximen PC-4 - A point the reference selects for this pattern.
- Shaochong HT-9 - A point the reference selects for this pattern.

Personalising the plan
- If the mind clouds, the pattern has progressed to the Pericardium and treatment escalates.
- If a rash appears, Blood-cooling herbs are increased.
- If fluids are badly damaged, moistening herbs are increased.
- If the fever is very high, medical assessment is arranged promptly.
Self-care
Diet
- Cool fluids in small sips if conscious
- Cooling moistening food in recovery: congee, pear
- Avoid spicy and warming food
Lifestyle
- Rest in a cool quiet place
- Have the fever medically assessed, especially if it persists or the mind is affected
- Monitor alertness and urine output
Qigong and movement
- Not during the febrile phase.
How it differs from look-alikes
Patterns that can be mistaken for heat in the ying level, and the contrast that separates them.
Heat in the Blood Level
Both are deep-level febrile patterns, but the Blood Level is more severe: it shows active bleeding such as nosebleeds, vomiting blood or blood in stool and urine, dark purple or black rashes rather than faint reddish ones, and often manic agitation or convulsions instead of the restlessness and insomnia of the Ying Level. Its tongue is deep crimson or purple, and it involves the Liver and Kidneys, not only the Heart and Pericardium.
Qi Level Heat
Qi Level Heat is shallower: a high constant fever not specifically worse at night, intense thirst for cold drinks where the Ying Level has little or none, profuse sweating, a red tongue with thick yellow coating rather than the crimson coatless Ying tongue, and a full overflowing rapid pulse rather than a fine rapid one. The mental state is irritable but conscious, where the Ying Level brings insomnia, confused speech or clouded consciousness.
Heart Fire blazing
Heart Fire is internally generated and not part of a febrile progression: tongue and mouth ulcers, a red tongue tip, irritability and insomnia, but no nocturnal fever, crimson coatless tongue or faint rashes. Its pulse is full and rapid rather than fine and rapid, and the person stays fully conscious.
Yin Deficiency
Yin deficiency with empty heat shares night sweating, warm palms and soles and evening fever, but it is a chronic gradual picture with low-grade afternoon fever and malar flush rather than an acute febrile crisis with high fever, mental disturbance and rapid deterioration. Its tongue is red with scanty coating but not as deeply crimson as in Ying Level Heat.

Safety and when to get help
For anything severe, sudden or spreading quickly, read when a skin problem needs urgent care.
This page describes a TCM pattern, not a diagnosis, and a pattern can accompany more than one skin condition. Get urgent medical help for a rash with fever, confusion or feeling very unwell; skin that is blistering, peeling or spreading quickly; swelling of the face, lips or throat, or difficulty breathing; or a lesion with increasing pain, warmth, red streaks or pus. For anything persistent, severe or changing, have it assessed by a clinician before treating it yourself.
Read the skin condition pages for the red flags that belong to a specific presentation.
Urgent-sign guidance is based on standard sources: NHS anaphylaxis, NHS cellulitis, NHS Stevens-Johnson syndrome, NHS melanoma.
Evidence note
No controlled trial of this treatment in skin conditions was identified for this entry. What is described here rests on the classical and later materia medica record and on traditional clinical use, not on controlled trial evidence, and it is not a prediction of benefit. Where skin-specific research does exist it is linked above.
Common questions
What does the Ying level mean?
It is the third of four levels in warm-disease theory. Heat at this depth begins to affect the Blood and the spirit, which is why the tongue turns crimson and sleep is disturbed.
Why does the fever worsen at night?
At night the Yang of the body withdraws inward, and Heat at the nutritive level is said to become more active then. This is why night fever is characteristic.
How is this different from Heat in the Pericardium?
By consciousness. Here the person is restless but oriented; in the Pericardium pattern there is delirium or loss of consciousness.
When should I see a doctor?
High fever with restlessness, confusion or a persistent fever needs medical assessment immediately.
